REHEARSAL QUESTIONNAIRE FOR POTENTIAL MEMBERS
Welcome! Since our choir does not have its own website yet, we are using our director’s secure website to post the questionnaire we also sent out by email. Filling out the form below will help us set our rehearsal schedule to include as many potential members as possible. Thank you for your help!

The following questionnaire will help us choose the best day & time for our choir rehearsals. Please take a few minutes to answer the questions below.
We hope we can find a solution that works for everyone, so the more information you can give us, the better.
We are aware that evening rehearsals can be problematic for some members who do not drive after dark. If we settle on evening rehearsals, we will also work with choir members to arrange carpooling for anyone who does not want to drive after dark—or for members who may not have access to transportation.
Rehearsals will be a combination of sitting and standing. If you have trouble standing for long periods of time, don’t worry, we’ll make whatever accommodations you need.
